Structural Family Therapy
Core Mechanism
Joining the family system, then actively restructuring dysfunctional boundaries and hierarchies through enactment, unbalancing, and boundary-making → reorganized family structure supports healthier functioning
Ontology
Symptomatic behavior is maintained by dysfunctional family structure — unclear boundaries, inappropriate hierarchies, and rigid or diffuse subsystem organization
Therapeutic Voice
"Instead of telling me about the argument, have the argument here. Show me what happens."
View of the Person
A person embedded in a family structure whose boundaries, hierarchies, and subsystems organize individual experience and behavior
Evidence
Components incorporated into guideline-recommended treatments (MST, FFT, BSFT)
Limited RCTs as standalone; incorporated into MST, FFT, and BSFT
Szapocznik et al. (2012) review of structural/strategic approaches
Enormously influential — structural concepts pervade all family therapy. Minuchin's direct, active style was revolutionary. Less practiced as pure model today but its concepts (boundaries, hierarchy, subsystems) are ubiquitous.
Conditions
Epistemology
Blind Spots
Therapist-as-expert model can be culturally inappropriate; hierarchical assumptions may not fit all family forms; less attention to individual intrapsychic processes; limited as standalone evidence base
Contraindications
Active domestic violence, active psychosis in the identified therapist's target for restructuring, families in acute crisis requiring stabilization before systemic restructuring, single-member families without relational system
Training
Graduate family therapy coursework + workshops in structural techniques
No single certification; Minuchin Center
Graduate coursework + workshops 24-40 hrs
$1K-3K
Philosophical Roots
Systems theory (Bertalanffy); cybernetics; Bateson (ecology of mind); Minuchin's own experience with immigrant families in New York; structural anthropology (Lévi-Strauss — deep structures organizing surface behavior)

Test Yourself
What is enactment in structural therapy?
Show answer
The therapist asks family members to interact with each other in session rather than talking about their problems — making the family structure visible in real time.
