9 clinical guideline checks
Read the recommendation and its scope. A source may discuss an approach without recommending it.
- Depression in adults: treatment and management (NG222)
Recommendation for the stated population
STPP is a named option using empirically validated depression protocols.
Scope: Adults with less or more severe depression
- Psychotherapie bij persoonlijkheidsstoornissen
Recommendation for the stated population
The Dutch guideline separately names short-term psychodynamic psychotherapy for avoidant personality disorder.
Scope: Avoidant personality disorder
- Clinical Practice Guideline for Management of PTSD and Acute Stress Disorder
Discussed in the source
VA/DoD gives psychodynamic therapy a neutral recommendation for PTSD. The category does not independently establish findings for every psychodynamic protocol.
Scope: The psychodynamic-therapy category for adult PTSD; no separate grade for this specific approach.
- Management of Major Depressive Disorder
Recommendation for the stated population
VA/DoD suggests short-term psychodynamic psychotherapy among seven unranked psychotherapy options (weak for). Treatment strategy also depends on severity, chronicity, prior response and preference.
Scope: Adults with uncomplicated MDD choosing psychotherapy.
- Management of Major Depressive Disorder
Discussed in the source
VA/DoD strongly recommends first-line evidence-based psychotherapy, referring to the list that includes short-term psychodynamic psychotherapy. This is a class-level recommendation, with no preferred individual approach.
Scope: Pregnant or breastfeeding adults with mild-to-moderate MDD.
- Clinical Practice Guideline for the Management of Chronic Multisymptom Illness
Insufficient evidence for or against
VA/DoD finds insufficient evidence for or against psychodynamic therapies for CMI with IBS symptoms. It discusses brief treatment courses but does not separately grade every psychodynamic protocol.
Scope: Brief psychodynamic treatments studied in IBS within the chronic multisymptom illness guideline.
- Clinical Practice Guideline for the Treatment of Posttraumatic Stress Disorder in Adults
Discussed in the source
APA reports insufficient evidence for this category. The statement does not separately evaluate every short-term psychodynamic protocol.
Scope: Adults with PTSD; the psychodynamic-therapy category versus no intervention or usual care.
- Clinical Practice Guideline for the Treatment of Depression Across Three Age Cohorts
Discussed in the source
APA lists psychodynamic therapies among comparable models; this does not separately grade every short-term protocol.
Scope: Adults with depressive disorders, excluding psychotic depression; the broad psychodynamic-therapy category.
- mhGAP: brief structured psychological treatment for depression
Recommendation for the stated population
WHO includes brief psychodynamic therapy among the structured psychological interventions that should be offered. The recommendation is strong, with moderate-certainty evidence.
Scope: Adults with moderate-to-severe depression, within the mhGAP non-specialist-care context.

Condition-specific assessments
Each conclusion applies to the population and use described. These source-based assessments do not certify the full entry or replace expert clinical review.
Anxiety Disorders
Population and scope: Adults with social anxiety disorder who decline cognitive behavioral and pharmacological interventions; a short-term psychodynamic protocol specifically developed for social anxiety.
Guideline recommendation
NICE CG159 says to consider short-term psychodynamic psychotherapy specifically developed for social anxiety when an adult declines cognitive behavioral and pharmacological interventions. The guideline explicitly notes more limited clinical effectiveness and lower cost effectiveness than CBT, self-help and medication. Its delivery recommendation describes a focused social-anxiety protocol addressing relationship themes, shame and engagement with feared social situations. This is a conditional alternative in a particular treatment pathway, not a first-choice recommendation for all anxiety disorders. It does not establish the same status for GAD, panic disorder, every brief psychodynamic variant or intensive short-term dynamic psychotherapy (ISTDP). The record reflects this 2013 guideline position, rather than an updated whole-literature efficacy comparison.

Personality Disorders
Population and scope: People with avoidant personality disorder receiving a specialist short-term psychodynamic psychotherapy protocol.
Guideline recommendation
The Dutch 2022 guideline recommends specialist STPP protocols for avoidant personality disorder. Its evidence discussion acknowledges scarce high-quality research for this subtype. This source-specific recommendation does not establish efficacy for every brief psychodynamic variant, ISTDP or all personality disorders.

Depression & Mood Disorders
Population and scope: Adults with uncomplicated major depressive disorder choosing psychotherapy.
Guideline recommendation
VA/DoD 2022 recommendation 7 weakly suggests STPP among unranked psychotherapy choices. This source-specific treatment recommendation does not establish superiority or transfer to every brief psychodynamic variant, including ISTDP.

How they work
Contextual Therapy
Core mechanism: Making visible the invisible loyalty bindings, relational debts, and ethical ledgers that govern family behavior enables renegotiation of intergenerational obligations and liberation from destructive entitlement patterns
Ontology: Human beings as fundamentally embedded in relational ethical contexts. Suffering often reflects intergenerational injustices and loyalty obligations that operate outside awareness.
Short-Term Psychodynamic
Core mechanism: Focused interpretation of core conflict + affective experiencing within the therapeutic relationship
Ontology: Unconscious conflict and maladaptive relational patterns maintained by defenses

What each assumes — and misses
Contextual Therapy
Philosophical roots: Buber (I-Thou; dialogue as the site of ethical obligation) is the direct and acknowledged influence; Levinas is a strong affinity rather than a documented source; existential philosophy of responsibility; intergenerational justice theory
Blind spots: No empirical research base; concepts can be difficult to operationalize; requires extensive training in systemic thinking; may not be accessible for clients seeking symptom relief
Therapeutic voice: Who in your family do you feel you owe something to? What do you feel you're owed?
Short-Term Psychodynamic
Philosophical roots: Freud (condensed); Ricoeur (interpretation as disclosure); Alexander & French (corrective emotional experience)
Blind spots: Pressure for speed may bypass clients who need longer relational repair; less suited for severe personality disorganization
Therapeutic voice: I notice you smiled just now when talking about something painful. What do you make of that?
Choosing between them
Contextual Therapy (Family Systems) and Short-Term Psychodynamic (Psychoanalytic) come from different traditions, which means they assume different things about what a person is, what causes suffering, and what the therapeutic relationship is for. The choice between them is often less about "which works better" and more about which set of assumptions fits the client and the therapist.
